DMAS Form Templates

Executive Summary

DMAS Form Templates is the setup screen where administrators review the templates used to build DMAS forms. The list shows each template with its type, publication and discontinuation details. Opening a template shows its name, description, and publication details, and lets you change the name and description, maintain its detail settings (fixed text, form context field tags, or a signature image), and review its tasks and maintain the interventions listed under each task. Templates cannot be added or deleted from this screen.

Key Features

  • List all DMAS form templates, newest first, with their type, added, discontinued, and published details
  • Open a template and change its name and description
  • Maintain the template's detail settings: text values, form context field tags, or an uploaded signature image
  • Review the template's tasks, grouped by category, with the number of interventions on each
  • Add, edit, and delete the interventions for a task

Typical Workflow

  1. Click the edit icon on a template (or right-click it and choose Edit) to open it.
  2. Click Edit at the top to change the Name or Description, then click Save.
  3. On the Details tab, use Add and Edit to maintain the template's settings. Choose a Content Type of Text, Form Context Field, or Signature, fill in the value that appears, and click Save.
  4. On the Tasks tab, right-click a task and choose Interventions (or click the interventions icon) to add, edit, or delete the interventions for that task, then click Close.
  5. Click Close to return to the list.
